Media Consent and Release Liturgical Services When a Baptism, Wedding or Funeral (“Sacrament”) occurs at Holy Trinity Greek Orthodox Church, upon the request of the individual or family, it is possible to broadcast the Sacrament using the Live-Stream system of the Church. It must be clear and understood that the Live Stream and Social Media are not private and can be viewed on the Internet by all. This signed release form is required by the principal parties in order to acknowledge and offer their consent. I certify that I have read this Media Consent and Release and fully understand its terms and conditions. I hereby release, discharge and hold harmless Holy Trinity Greek Orthodox Church and its officers, employees, representatives, agents, volunteers, successors and assigns from any and all claims arising out of or in connection with the creation and use of the aforesaid materials.
